Skin excoriation
Incontinence is recognised as a factor which can cause soft tissue damage by excoriating the skin through prolonged contact with urine and faeces.

A prerequisite to pressure ulcer grading is to first of all correctly identify the cause of tissue damage in order that care may be planned appropriately.

Damaging factors can include:
  • exposure to excess moisture
